In consideration for my attendance and participation in the martial arts training offered by Sulphur Springs ATA Martial Arts (hereafter known as SSATA), I, the student/parent, acknowledge the existence of certain inherent risks in this type of training and hereby agree to assume all risks. I further relieve the school, its management, assigned staff, and fellow students from any and all liability resulting from loss, whether personal belongings or bodily injury. I also hereby state, that myself for my child is physically fit to take the prescribed course of instruction and do so of my own free will in exchange for an agreed upon fee.  

I, the undersigned member agree to purchase a membership from SSATA and agree to pay for such membership dues in accordance with the terms set forth in this agreement. this membership is subject to SSATA rules and regulations, and hours/facilities may vary based on demand.  My failure to attend regularly and utilize center facilities does not relieve me of my obligations, regardless of circumstances.  Payment of monthly tuition will be handled through a recurring billing agreement with KickSite/EPayUSA.  I understand that there is a NO REFUND policy on any monies I will pay to SSATA.  Returned check fee is $30.  EFT/Auto Charge through KickSite/EPayUSA will only be stopped or placed on hold upon receipt of a written cancellation/hold notice from the responsible party.  Please allow up to 3 business days to stop or hold auto payment.  Your Payment will appear as Sulphur Springs ATA on your bank or credit card statement.  

I further understand that my (or my child's) likeness may be used in photographs and video form, and distributed in social media and advertisements at no compensation.  

Sulphur Springs ATA Martial Arts is an independently owned and operated licensee of the American Taekwondo Association.  ATA International is not a party to Licensee's tuition agreements.
